When requested by the County Social Service Agency, Human Service Center or the Division, any of the following information and verifications must also be provided:  

  1. Fire inspections by the state fire marshal or local fire inspector
  2. Physicians examination
  3. Psychological examinations
  4. Proof of age and relationship of applicant and/or individuals residing in the home
  5. Sanitation and safety inspection reports
